This Monthly Membership Agreement (“Agreement”) is entered into between Athletek LLC (“Facility”) and the undersigned member (“Member”). By signing this Agreement, Member agrees to the terms and conditions outlined below.

1. Membership and Billing

Member is purchasing a monthly membership to use the Facility’s gym and training areas in accordance with all Facility rules and policies. The monthly membership fee will be billed automatically to the credit card or debit card provided by Member at the time of registration. Membership fees are billed in advance each month and are non-refundable once charged.

2. Auto-Renewal and Member Responsibility

Membership will auto-renew each month and continue indefinitely until Member cancels in accordance with this Agreement. It is the Member’s sole responsibility to cancel the membership.

3. Membership Cancellation

To cancel, Member must submit a written cancellation notice via email to [email protected]. Cancellation notices must be submitted at least 30 days prior to the desired cancellation date. Any charges scheduled within 30 days of receipt of the cancellation notice will still be billed.

4. Member Acknowledgment 1. Member understands and agrees that: Charges will continue indefinitely each month until a proper cancellation notice is received. Failure to cancel in accordance with this Agreement will result in continued billing. No partial month refunds or credits will be issued.

5. Waiver and Release of Liability

Member acknowledges that participation in gym and training activities involves inherent risks of injury. Member voluntarily assumes all risks and agrees to release and hold harmless Athletek Baseball, LLC, its owners, employees, and agents from any claims or liability arising from use of the Facility, except in cases of gross negligence or willful misconduct.

Entire Agreement

This Agreement constitutes the entire understanding between the parties and supersedes any prior representations or agreements. Athletek Baseball, LLC reserves the right to update Facility rules or policies at any time, which will be communicated to members
